I can't make it past a mini game in chapter 5 (with the doll heads). After I finish the 2nd stage the game immediately crashes and I get alicemadness.exe has stopped responding.

No, this is the retail release without the first game. Although it's possible to copy Alice1 folder from someone, it's under 1gb.
Then it could be launched from Alice: Madness Returns after you edit the DefaultEngine.ini file. Or with commandline parameters:
-INT -RunningFromAlice2

It's true, sorry. If it detects your locale is one of those from multi6, it won't display the dialog. You can try to temporary change your windows locale to force it to be displayed.
Otherwise, you can try to change the language of the installed game by modifying the following registry key:
HKEY_LOCAL_MACHINE\SOFTWARE\EA GAMES\Alice Madness Returns\Locale
The value is "en-us" for english installation.
